Before Your First Visit:  You can make your child's first visit enjoyable and positive by putting your child in the proper frame of mind.  Tell your child in advance that someone is going to look at their teeth.  It is best if you refrain from using words around your child that may cause unnecessary fear, such as: needles, pull, drill or hurt.  Children's Dental Office makes a practice of using words that convey the same message but are pleasant and non-frightening to the child.  Comprehensive dental services are available for the child with special needs.

New Patient Form:  For your convenience when visiting our office, please print the New Patient Health History Form, complete the information and bring it with you to your first visit.

To print the form you will need Adobe Acrobat Reader.  If you do not have Adobe Acrobat, please click here to download it for free to your computer.

Appointment Scheduling:  Appointment times are designed to fit your child's dental needs and help to ensure a positive dental experience.  We make every effort to provide you with a convenient appointment.

  • Throughout our years in practice, we have found it beneficial for small children (under the age of 7) to be seen earlier in the day.  We can work more slowly with the child for their comfort.

  • We are aware of school policies which make it more difficult for patients to be out of school for any reason.  However, dental appointments are an excused absence with a doctors school pass, which we will be happy to provide.

  • We would like to be able to see all school age patients after school.  However, this is not possible.  Therefore, to make certain everyone has a fair share of scheduling, we suggest alternating after school and during school appointments.  Please help us to help your child and others by not asking for exceptions.

  • Coming late for an appointment may require rescheduling.  We do not keep others waiting.  Please call if you are going to be late.

  • Because we feel it is important to provide a higher lever of service and a more efficient means of communication, we provide our patients with the courtesy of a timely and efficient personalized appointment reminder system called "House Calls."  This system will call you at home two days prior to your next appointment.  To ensure that you receive your appointment reminder, we will schedule House Calls to phone you between the hours of 5:00 p.m. and 8:00 p.m.  After you hear your appointment reminder message, House Calls will prompt you to confirm the appointment.  You will also have the option of leaving us a message.  If you are not available, the system will leave you a message on your answering machine.  As always you can contact our office during our regular office hours.

  • Missed Appointments - unless canceled 24 hours in advance, our policy is to charge for missed appointments at the rate of a normal office visit.  Please help us to serve you better by keeping your scheduled appointment.

Finances:  Payment for professional services are due at the time dental treatment is provided.  We accept cash, checks, ATM cards, Visa, MasterCard, Discover, and American Express.  We offer an extended payment plan with prior credit approval.  Payment plans may be set for patients without dental insurance if more than $300 of treatment is required.  Payment can be made through CareCredit or personal arrangements with your bank or similar lending institution.  If a payment plan is agreed upon, you will be asked to sign a financial agreement.  Payments are due when statements are received.

Dental Insurance:  If we have received complete insurance information on the day of your appointment, we offer the courtesy of filing your dental claims electronically.  You must be familiar with your insurance benefits, as we will collect from you any estimated amounts not covered by your dental plan.  You are responsible for any balance on your account after 45 days, whether insurance has paid or not.  If insurance is pending you will receive an interim statement to let you know that the account has not been paid.  Please understand we file your claim for you as a courtesy.  Your insurance is a contract between you and the insurance company.  We are not responsible for how your insurance company handles its claims or for what benefits they pay.  Benefits are not determined by our office.  Insurance companies set their own fee schedules, and are usually determined by how much you or your employer has paid for coverage or the type of contract your employer has set up with the insurance company. Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area.  You are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary.

Most importantly, please keep us informed of any insurance changes such as policy name, insurance company address or a change in employment.
